Because family physicians prescribe power mobility devices rarely more than two or three times annually the Texas Acadamy of Family Physcians developed a clinical guide to assist in evauating a patient for a power mobilty device. The purpose of the guide is to assist the treating physcian and assure conformance with Medicare’s extensive documentation requirerments and face-to-face examination. This guide is not a Medicare-approved from. Before using the form, physcians are encouraged to review the DME MAC physician guidance letter, which provides a synopsis detailing the highlights of documentation requirements.
